Main responsibilities

主要职责 在主管医生授权下,完成以下工作,积极建议、主动沟通、及时反馈,完成各项评估Under the authority of the supervising doctor, complete the following tasks, actively advise, proactively communicate, provide timely feedback and complete all assessments 1) 掌握失能等级评估,需要主管医生确认;包括入住前初步评估、入住后确认评估、即时评估、每半年更新评估。根据评估结果及机构的接待能力,相应填写内部信息联络表,做好入住长辈信息与各部门的沟通;Keeping abreast of the assessment of the level of disability, which needs to be confirmed by the supervising doctor; including initial assessment before admission, confirmation assessment after admission, immediate assessment and half-yearly update assessment. Complete internal information contact forms accordingly, based on the assessment results and the institution's capacity to receive them, and communicate information about the resident elders to all departments; 2) 老年综合评估及更新:入住后一周内、老人情况发生重大变化时、每半年更新。Comprehensive geriatric assessment and update: within one week of admission, whenever there is a significant change in the elderly person's condition, every six months 3) 参与约束评估及医嘱更新:新入住老人约束讨论、约束医嘱开具、每月重新评估开医嘱。Participate in restraint assessments and medical advice updates: new admissions restraint discussions, restraint orders, monthly reassessments and medical advice 4) 参与辅具评估及医嘱更新:参与新入住老人康复评估、多学科讨论、医嘱开具、每半年及必要时更新。Participation in assessment of aids and updating of medical prescriptions: participation in rehabilitation assessment of new admissions, multidisciplinary discussions, prescriptions and updates every six months and as necessary. 5) 根据老人失能及失智情况,参与多学科团队讨论、评估辅助医疗目标、开具辅助医疗目标医嘱,每半年或即时更新;Participate in multidisciplinary team discussions, assess paramedical goals and prescribe medical advice on paramedical goals based on the elderly person's disability and dementia, with semi-annual or immediate updates; 6) 熟悉系统内所有相关评估量表:包括但不限于:MMSE、NPI、GDS、ADL/IADL、MNA、Braden压伤评估表 、起立-行走计时 、Tinetti量表 、疼痛评估相关量表。Knowledge of all relevant assessment scales within the system: including but not limited to: MMSE, NPI, GDS, ADL/IADL, MNA, Braden Pressure Injury Assessment Scale, Rise and Walk Timing, Tinetti Scale, pain assessment related scales. 7) 新入住老人需要完善老人医疗资料:病症、过敏史、饮食、风险、补充信息、生前预嘱(根据情况)、体检报告等,妥善保管。Newly admitted seniors need to complete their medical information: medical conditions, allergy history, diet, risks, additional information, living will (as appropriate), medical reports, etc. and keep them safe. 8) 开具相应医嘱:药物及管理、监测、饮食、辅助医疗、辅具、约束、营养补充剂等。Prescribe appropriate medical advice: medication and management, monitoring, diet, complementary medicine, aids, restraints, nutritional supplements, etc. 9) 根据自行管理药物所需评估及约束讨论结果,根据流程及时与老人或家属沟通并签署相关同意书。Based on the results of the assessment and restraint discussion required for self-administered medication, the elderly person or family member is communicated with and signs the relevant consent form in a timely manner according to the process. 10) 每日查房,根据老人情况,随时掌握老人健康水平与病情动态,确保老人得到正确照护;及时更新医嘱、补充交班、观察及诊疗记录、报告上传、家属沟通等;紧急情况需与主管医生沟通确认;并在系统中正确及时记录。Daily check-ups to keep track of the health level and condition dynamics of the elderly according to their condition and to ensure that they are properly cared for; timely updates of medical orders, additional shifts, observation and treatment records, report uploads, family communication, etc.; emergencies need to be confirmed by communication with the doctor in charge; and recorded correctly and timely in the system. 11) 外部会诊:根据老人及专业需求,提出会诊建议,整理会诊人员名单,提前准备会诊资料,参与会诊过程,记录会诊意见,与家属沟通会诊意见。External consultation: Based on the needs of the elderly and the profession, we make recommendations for consultation, compile a list of consultation participants, prepare consultation materials in advance, participate in the consultation process, record the consultation comments and communicate with the family about the consultation. 提升专业能力、增强团队协作Enhancing professional competence and teamwork 12)熟悉并不断学习老年相关专科知识,比如安宁疗护、认知症、非药物疗法等。Knowledge and ongoing study of geriatric-related specialties such as hospice, cognitive disorders, non-pharmacological therapies, etc. 13)具有良好的沟通能力和团队协作能力,与老人和家属保持良好沟通。Good communication and teamwork skills to maintain good communication with the elderly and their families. 14)落实员工培训计划,及时通知医护人员相关的培训信息,具备培训能力,根据机构培训规定,在主管医生安排下完成员工培训。Implement staff training plans, inform healthcare staff of relevant training information in a timely manner, have the ability to train, and complete staff training in accordance with institutional training regulations and under the arrangement of the supervising doctor 15)

与护理团队协作,参与多学科团队讨论,完成老人个人生活计划、护理计划制定并及时补充医疗部分。Collaborate with the care team and participate in multidisciplinary team discussions to complete individual life plans for the elderly, develop care plans and supplement medical components in a timely manner. 16)根据相关管理规定, 负责老人药品处方管理,并监督指导药师、护士严格执行;新增药品及时添加到表格备案。In accordance with the relevant regulations, we are responsible for the management of geriatric drug prescriptions and supervise and instruct pharmacists and nurses to strictly implement them; new drugs are added to the form in a timely manner for record. 17)做好老人预防保健,健康宣教。Preventive health care for the elderly and health promotion.

协助主管医生完成相关工作Assisting the supervising physician with related duties 18)促进并完善机构老年医学领域合作资源,组织并参与老年医学协调委员会相关资料准备。Promote and improve resources for institutional collaboration in the field of geriatrics and organise and participate in the preparation of materials related to the Geriatrics Coordinating Committee. 19)协助主管医生准备年度医疗活动报告。Assist the supervising physician in the preparation of the annual medical activity report. 20)配合主管医生,参与部门质量审核和评估;包括辅助医疗报告书写规范与否。In conjunction with the supervising physician, participate in departmental quality audits and assessments, including the standardisation and non-standardisation of paramedical report writing. 21)参与多学科会议和家属访谈:了解老人整体疾病及风险,提出建议,与家属沟通,做好记录。Participate in multidisciplinary meetings and family interviews: understand the overall illness and risks of the elderly, make recommendations, communicate with the family and keep records. 22)与主管医生协调24小时医疗备班。Coordinate with the supervising doctor for 24-hour medical preparation. 23)完成主管医生交代的其他临时性工作。Complete other ad hoc tasks as assigned by the supervising physician. 资历及经验要求 1. 临床医学专业本科及以上学历。

• 有效注册的医师执业证书和资格证书。
• 医学、临床、专业本科以上学历 4. 三年以上临床经验 5. 老年科、全科、神经内科背景,熟悉老年综合评估、认知症照护、安宁疗护及老人突发事件的紧急处理;
