职位概述

护士提供医疗照护服务并参与协调院内照护团队、辅助医疗人员和外部人员的工作。Nurse provides nursing services and participates in the coordinating work of the facility's nursing team, auxiliary medical staff and external staff.

主要职责

组织照护和活动Organize nursing and activities

• 参与并协助各部门的工作。Participate and assist in the work of all the department.
• 合理安排老人的活动和照护计划,并做好相关记录,包含但不限于照护系统和纸质文件。Do reasonable arrangements for the residents' activities and care plan, and do a good job of related records, include but not limited the operation system and paperwork.
• 参与制定整合性护理计划。Participate in the development of integrate care plan.
• 及时更新护理文件(特别是墙上计划表)Update the nursing document in time (especially the plan on the wall).
• 确保老人得到正确的照护Ensure proper care for the residents.

照料老人Care for the residents

• 接待、倾听、通知、支持和陪伴老人及其家属Receive, listen, inform, support and accompany the residents and their families.
• 保持与家属之间的信息沟通的持续性,如入住老人住院,护士需定期了解老人情况并告知家属 发现问题(如住院),及时通知家属Maintain the continuity of information communication with their families, such as the residents' hospitalization, the nurses need to regularly ask the situation of the residents and inform their families of the problems (such as hospitalization), and inform the families timely.
• 参与老人的综合评估(失能评估表,如XX集团Linda评估表等)Participate in the residents' comprehensive assessment(disability assessment, such as Orpea Linda assessment, etc.).
• 建立、填写并跟踪老人个人信息监测表Establish, fill in and track the residents' personal information monitoring form.
• 完成技术性的护理工作。Complete the technical nursing work.
• 监控老人的健康状况和社会行为能力,发生任何健康问题必须及时告知医生。Monitor the health status and the ability of social behavior of the residents, if find any health problems, must report to the doctor timely.
• 及时掌握老人的化验结果,发现异常及时通知医生。Master the test results of the residents in a timely manner, if find any abnormal situation, should report to the doctor timely.
• 定期监控老人体重,保证老人遵守饮食计划,确保老人摄入所需营养(填写监控表格)。Keep regular monitoring of the residents' weight, ensure that the resident complies with the diet plan and intakes the needed nutrition (to fill in the monitoring form).
• 认真执行评估量表(疼痛、跌倒、营养状况等)。Keep strict implement of the assessment scale (pain, fall, nutritional status, etc.)
• 避免对老人进行约束,预防大小便失禁。Avoid restraint to the elderly, prevent incontinence.
• 监督并参与老人个人卫生和生活舒适度相关的护理服务(如厕、转移、安置、压疮的预防等)。Supervise and participate in the resident's individual health and living comfort degree related nursing services (toilet, transfer and resettlement, pressure ulcer prevention, etc.).
• 检查老人生活环境并按照老人的失能程度进行相应调整。Check the residents' living environment and adjust in accordance with the disability degree of the residents.
• 及时填写并更新老人档案。Fill in and update the resident's' file timely.
• 保证养老机构的各项规范得到遵守。Ensure strictly comply with the operating requirements of the facility.
• 自觉遵守并监督他人遵守安全规范。对存在风险的老人进行跟踪监控。Observe and supervise the others to comply with the safety regulations. Track and monitor the residents have risk.
• 一旦发生紧急情况,立即通知医生、评估老人的状况,并在医生到达之前实施必要的急救措施。In case of an emergency, notify the doctor to assess the condition of the residents, and implement the necessary first aid measures before the doctor arrives.

管理和定期检查设备、耗材,预防院内感染Management and regular inspection of equipment, supplies, prevent internal infection

• 确保医疗照护和急救器材能够正常使用。Ensure the normal use of nursing and first aid equipment.
• 做好医疗照护器械及各区域卫生和消毒工作。Do a good job of cleaning and disinfection of nursing equipment and all the related areas.
• 确保高危用品的正确使用和流通,尤其是感染性医疗废物的处理。Ensure the proper use and distribution of high - risk products, especially for the treatment of infectious medical wastes.

药品流通的追溯、管理和控制Traceability, management and control of drug circulation

• 正确执行医嘱(治疗、照护、检查、饮食、康复等)Execute the doctor's instructions properly(treatment, nursing, inspection, diet, rehabilitation, etc.).
• 合理管理和存放药品Keep proper management and storage of drugs.
• 做好药品库存的管理工作,保证药品在有效期内。Do a good job in the management of drug inventory, ensure that the drug is within the validity period.
• 保证配有常用药品并及时检查和补充。Ensure the storage of commonly used drugs and inspect and supplement timely.
• 确保护士站和治疗室医用冰箱的温度和卫生。Ensure the temperature and hygiene of medical refrigerators in the nursing stations and the therapeutic room.
• 为老人正确无误地配药和分发药品。Keep correct dispensing and distribution of drugs for the residents.
• 及时准确核对医嘱,如有疑问及时通知医生。Check the doctor's advice timely and accurately, if there is any doubt, inform the doctor in a timely manner.
• 定期检查急救箱内的器材和药品,及时补充。 Keep regular inspection of first aid box's equipment and drugs, and supplement timely.
• 根据现行规定管理和存放精、毒、麻及高危药品,保证此类药品的使用可追溯。According to the existing provisions, manage and store the alcohol, poison, anesthetic and high-risk drugs, ensure that the use of these drugs can be traced back.

内部沟通及可追溯性Internal communication and traceability

• 做好照护资料的记录和管理。Do a good job of nursing data records and management.
• 确保各项护理操作和护理记录(白/夜班记录表、监测表、交班表等)的可追溯性,做好口头和书面交班。Ensure all the nursing operation and nursing records' traceability (white / night shift record, monitoring form, shift handover form, etc.), do a good job of oral and written shift handover.
• 参与并协助各部门的工作。Participate and assist in the work of all the departments.

与夜班工作相关的工作Night shift related work

• 如有需要,接听电话If necessary, answer the call.
• 特别关注机构内安全隐患,如火灾隐患等。Pay special attention to the internal security risks, such as fire hazards, etc.
• 出现紧急情况时,立即呼叫值班人员并采取相应措施。When there is an emergency, call the personnel on duty immediately and take the appropriate measures.
• 夜间工作时,保持低调安静,尽量不打扰到老人休息。While working in the night, keep quiet, try not to disturb the residents' rest.

Qualification And Experience Required
资历及经验要求

全日制护理专业大学专科(三年制)以上学历。 At least 3 years' full time junior college diploma for Nurse major. 有效注册的护士执业证书。 Valid Nurse Certificate of Practicing and the location of practicing can be changed to Keppel facility. 具有丰富的一线护理工作经验,有养老机构护理护士从业经验者优先。

Stronger experience in clinical nursing, prefer to own the experience working in senior care, long-term care, or nursing home.
